How we provide services
Our personalised health system navigation service and community support network can help you and your loved ones live well, stay connected, recover from illness, and age confidently — all shaped by you. Discover how we provide services.
The client is at the centre of all activity
- The client, you, is the one with the choice - and the one whose rights we uphold.
- The client is the reason for and focus of our work.
- The client is entitled to the best care possible, in your home, in Wagin.
- Services available to the client are clinical supports (to maintain or regain capabilities), independence (help in managing to overcome loss of skills) and support in everyday living (keeping your home liveable and safe).

- Co-operative Care Wagin (CCW) sets up supports and services for clients in line with the support plan and budget.
- We provide ongoing day-to-day co-ordination and navigation.
- We engage verified support workers and service providers and we ensure the quality of this work.
- We review shift notes.
- We are the first point of contact for customer enquiries.
