Health Navigation Service
At the heart of Co-operative Care Wagin is our Health Concierge Service – a welcoming, judgment-free resource where no question is too silly, too big, or too small.
Our work on your behalf includes:
Personalised navigation
Help connecting and linkage with health and social services
Systems support
Warm, in-person assistance navigating complex healthcare systems (including home care packages), using IT, telehealth, or other means
Advocacy
Working together with you and service providers to bring necessary services to Wagin
Community Support Network
We're building a "village awareness" approach where community members support each other to be valued during health recovery and to age in place. This includes:
Transport assistance
Help getting to medical appointments
Social connections
Combating isolation through community engagement
Practical support
Connecting members with helpers for challenging household tasks
